Directions for Nominations:
1. Read the Monarch Award criteria on this website http://www.islma.org/monarch.htm
2. Books and authors on the 2009 Master List or previous lists are ineligible.
3. Submitting nominations on the ISLMA/Monarch website is the preferred method of nomination. Use this form to collect nominations until they can be submitted.
4. If online submissions are not possible, please print all nominations on this official nomination form. This form may be copied or printed from the website. Mail nominations to the address below. Faxed nomination forms will not be accepted.
5. Include author, title, publisher, copyright date and type for each nomination. Types are: picture book, easy reader, or chapter book.
6. Only titles with complete information will be considered for nomination.
7. Deadline for submissions for the 2009 Award is September 15, 2008. Nominations received after this date will be considered for the 2011 Award if they meet the criteria.
