Directions for Nominations:

1. Nominations may be submitted on the ISLMA website, www.islma.org. This is the preferred method of nomination. This nomination form may be used to accumulate nominations before submitting them.

2. If online submissions cannot be made, please type or print all nominations on the official nomination form. If needed, the form may be copied. Mail to the address included on the form. Faxed nomination forms will not be accepted.

3. Include author, title, copyright dates and publisher for each nomination.

4. Deadline for submissions is October 15, 2009.
